The genesis of QRS changes produced by selective coronary arteriography.

چکیده
Selective coronary arteriography produces characteristic ST-T changes associated with significant axis shifts. In patients with normal control AQRS (mean, +88.60; SD -t12.20) left coronary injections induced an important leftward deviation of the AQRS: mean, +8.2.; SD, +49.10 (P < 0.001). Right coronary artery opacification changed the AQRS from a control mean of +66.50; SD, +17.20 to a mean of 89.3°; SD, +16.70 (P < 0.001). Left axis shifts after left coronary artery injections were significantly more marked than right axis deviation after right coronary injections. Important increases (beyond 0.02 sec) in ventricular activation time were not observed. These electrocardiographic abnormalities were attributed to the transient ischemic or toxic effect produced by the bolus of dye. This could result in different degrees of complete or incomplete block in the divisions of the left branch or in an extensive peripheral block affecting large areas of the subendocardial regions of the heart.

Selective cine coronary arteriography correlated with the hemodynamic response to physical stress.
The degree of radiologic abnormality of the coronary arterial circulation due to atherosclerosis, as detected by selective coronary cine arteriography, correlated well with certain hemodynamic changes in patients undergoing a measured amount of supine leg exercise. Notable among these were a rise in pulmonary artery pressure above the normal response level and cardiac index below the expected l...

It is a common Knowledge that the ECG diagnosis of myocardial infarction in the presence of left bundle branch block (LBBB) is extremely difficult and often impossible. Physicians frequenlly recommend that patients with chest pain in the presence of LBBB receive thrombolytic therapy or urgent coronary arteriography assuming that acute injury and ischemia cannot be interpreted in the presence o...
متن کاملSelective coronary arteriography.
The technique of selective coronary arteriography, as described originally by Sones, was employed in 255 patients. Successful catheterization of both coronary arteries was carried out in 88% of these patients, and in the last 100 examinations both coronary arteries were entered in 95 patients. Selective coronary arteriography is a useful diagnostic tool but is a potentially hazardous form of ex...
